Depression Screening
Please answer the following questions based on how you have been feeling over the past two weeks. This assessment is based on the PHQ-9, a validated depression screening tool.
1.Over the past two weeks, how often have you felt sad, down, or depressed?
2.Over the past two weeks, how often have you lost interest or pleasure in activities you usually enjoy?
3.Over the past two weeks, how often have you felt tired or had little energy?
4.Over the past two weeks, how often have you had trouble falling asleep, staying asleep, or sleeping too much?
5.Over the past two weeks, how often have you experienced changes in appetite (eating too much or too little)?
6.Over the past two weeks, how often have you felt bad about yourself or that you are a failure?
7.Over the past two weeks, how often have you had trouble concentrating on things like reading or watching TV?
8.Over the past two weeks, how often have you moved or spoken so slowly that others noticed, or been so fidgety or restless that you have been moving around more than usual?
9.Over the past two weeks, how often have you had thoughts that you would be better off dead or of hurting yourself in some way?
